Rather than take a deposit that would have been refunded at the event, we will only take a payroll deduction of $20 if you FAIL to show up for your scheduled appointment.

MISSED EVENT PROMISSORY NOTE

1. THE PARTIES.
This Unsecured Promissory Note (“Note”) is made by the undersigned Team Member (“Borrower”) in favor of Partner Veterinary Services / applicable Hospital Location (“Lender”).

2. PURPOSE & AMOUNT.
Borrower acknowledges responsibility for a $20.00 charge related to a missed tattoo event.

3. REPAYMENT TERMS (PAYROLL DEDUCTION).
Borrower authorizes Lender to deduct $20.00 from Borrower’s next regularly scheduled paycheck immediately following the missed event. Borrower understands and agrees that this deduction will be taken from wages permitted by applicable law and that gross pay will remain above minimum wage.

4. DEFAULT & ACCELERATION.
If repayment is not successfully collected from the next paycheck for any reason, Borrower agrees that the amount becomes immediately due and payable, and Lender may pursue any lawful means of collection.

5. WAIVER.
Borrower waives presentment, notice of dishonor, and any requirement of additional notice prior to payroll deduction.

6. GOVERNING LAW.
This Note shall be governed by the laws of the States of Virginia and Maryland.

7. ENTIRE AGREEMENT.
This Note represents the entire agreement regarding this obligation and may not be modified except in writing.
